ADVANCE HEALTH CARE DIRECTIVE

HOW DOES AN AHCD WORK?

This document plans for those unthinkable moments of medical emergencies, comas, and hospice.  It allows you to designate an agent or agents who will ensure that you receive the medical treatment, convalescence, and end-of-life care you want even when you are unable to speak. 

 

Additionally, you are able to make your wishes known for if the worst should happen if you want to make organ donations, have an autopsy, and designate whether you want to be cremated or interred.

The purpose of this document is to prevent your loved ones from having to go to court to obtain a conservatorship over your person should you ever become unable to handle your own medical decisions.

This document is available to individuals: 18+ years of age.
